High Quality, Personalized, One-on-one care

BlumeHaus Physical Therapy is out of network ​with all insurance companies. This allows us to ​provide one-on-one personalized care without ​constraints of an insurance company. More time ​with your physical therapist without technicians ​or aides allows you to get better faster and with ​less visits.

Many insurances have out of network benefits. ​We can provide you a superbill to send to your ​insurance for possible reimbursement. We also ​accept HSA/FSA card for payment.

Packages and Cancellation Policy

PACKAGES

A package of discounted treatment sessions are offered after completion of your iniatial evaluation. ​Choose from a package of 6 or 12 sessions. Packages are non-refundable, non-transferrable, and expire ​one year from date of purchase.

How do I know how many sessions I need?

At your initial evaluation, your physical therapist will make a recommendation based on your needs and ​goals.

CANCELLATIONS

All cancellations must be made 48 hours prior to your appointment. If you cancel within the 48 hours of ​your scheduled session, you will lose one session off your package.
